New Pharmacy Steps and Guidelines
To operate a pharmacy, one must first apply for a Certificate of Need (CON). The first step is to submit a letter of intent, a CON application, and a $200 application fee payable to the Government of the VI. You must submit the original plus 3 copies of everything. If approved, you need to apply to the VI Board of Pharmacy for a permit to operate a pharmacy; the forms and applications are listed below.

All Physicians applying for a permanent VI Physician license are required to create a login for access, then complete the following process via The Federation of State Medical Boards website links provided below

Allied Health Clearance
NEW: If you are applying for a DLCA business license and your occupation/specialty does not have an established health professional board; you need to complete an Allied Health Application for DLCA clearance. Please pay close attention to the check list of required items noted on page 5 of the application.
RENEWAL: Complete the Allied Health Renewal Form and be sure to attach the required documents.
Certificate of Need (CON)
NEW CON APPLICATIONS:
Title 19 VIRR §225-3 Certificate of Need Program; activities for which certificate is required
No person, organization, or governmental unit, acting severally or jointly with any other person, organization or governmental unit shall establish, construct, or expand a health facility or health service, or incur capital expenditure on behalf of a health facility or health service, or acquire major medical equipment in the Virgin Islands without applying for and receiving a Certificate of Need.
The steps required to obtain a CON are outlined the CON Process Letter. To apply, submit of a Letter of Intent, Official CON Application with supporting documentation and an application fee ($200- service requests or $400 facilities) payable to “Government of the VI”. You must submit an original package plus 3 copies. PLHP staff is available to meet with applicants to discuss the requirements and provide further guidance.
RENEWAL CON/LICENSE REQUIREMENTS:
Your CON License Renewal may be considered for renewal upon your request in writing, to the Commissioner of Health two (2) months before the expiration date.
Submit a CON Annual Report with all the required supporting documentation.
